Five facilities received Type A violations

Some Central Illinois nursing homes were cited by the state’s Department of Public Health for violations.
Rose Care Center of Peoria, Sharon Health Care Elms and Washington Christian Village, also in Peoria, were cited, along with Meadows Mennonite Home in Chenoa and Morton Terrace H&R Centre.
According to a quarterly report of nursing home violators, all of these facilities received Type A violations, meaning there is a chance that someone could die or has died, including causing serious mental or physical harm.
